Dr. Miller spends his Fridays fishing or teaching at the dental school in Birmingham.
One of Dr. Miller’s pet peeves (our staff says he has a couple) is to wait at a doctor’s  office. He carries this over to our practice. We try to make sure that we get to you on time. Referral of emergency patients can cause delays to the schedule, and your understanding and patience are very much appreciated.
Please assist us at the time of your initial visit to the office by providing the following :
X-rays or records from your dentist.
List of medications you are currently taking (please note that you can fill out your patient registration and medical history form on this web page).
Insurance cards or forms
A guardian if you are under 18.
Information on medical conditions such as artificial valves or joints or heart murmurs wich may require premedication
If your referring dentist has taken X-rays, you may request that they be forwarded to us.
